Chinese traditional medicine Dan Shen compound for acute myocardial infarction

Abstract
Background: Chinese Traditional Medicine Dan Shen compound for acute myocardial infarction (AMI), which is the leading cause of death in the western world, is one of the most common prescriptions of Chinese clinicians.

Objective : The objective of this review was to assess the effects of Chinese traditional medicine Dan Shen compound for patients with a diagnosis of AMI.

Search Strategy: We searched MEDLINE, EMBASE, Chinese BioMedical Database (CBMD), Cochrane Central Register of Controlled Trials, reference lists of articles and the authors personal collections up to 2003.

Selection Criteria: Randomised controlled trials comparing Chinese Traditional Medicine Dan Shen compound with Western medicine, or comparing Integrated Chinese Traditional Medicine Dan Shen compound and western medicine with western medicine in AMI. Data collection and analysis: Two reviewers extracted data and assessed trial quality independently. Authors were contacted for missing data. Results: Twelve trials involving 2881 patients were included in the primary analysis. The quality of the trials was variable. Four trials involving 282 people compared Chinese traditional medicine Dan Shen compound with routine treatment. Compared to routine treatment, there is no significant difference on mortality (RR0.33, 95%CI0.07~1.58). But, Dan Shen compound significantly improved the rate of clinical recovery (RR2.03, 95%CI1.09~3.79) and the rate of vessel re-open (RR1.98, 95%CI1.41~2.77). Seven trials involving 1249 people compared routine treatments and Dan Shen compound with Routine treatment. The latter significantly reduced the mortality (RR0.43, 95%CI0.30~0.61), and improved the rate of relief in chest pain and distress on the first day (RR6.00, 95%CI2.45~14.68) and arrhythmia (RR2.11, 95%CI1.24~3.59). One trial involving 1350 people compared Dan Shen injection with routine treatment, and there is no significant difference on mortality (RR0.78, 95%CI0.58~1.03). Conclusion: Integrated Dan Shen compound and western medicine appeared more effective than western medicine on reduction of the mortality in AMI.
