Framework for analysis of the factors influencing the adoption of new technologies in health care

Abstract
Introduction: Exponential adoption of health care technologies has been associated with the continued growth of health sector costs. Decisions regarding adoption of technologies for health care involve numerous levels and stakeholders
Methods: Scientific literature with keywords and [All Fields] "adoption of innovation" OR "knowledge utilization" OR "technology transfer" AND "technology/biomedical" , indexed from 1960 until March 2006, were retrieved from MedLine/PubMed, EMBASE, LiLaCS/BIREME and CAPES Portal/Ovid Journals Full Text/FMUSP. Inclusion criteria included relevant theoretical contribution and description of factors influencing decisions to adopt new health care technologies.
Results: This study summarizes the relevant literature, discusses the factors influencing health technology adoptions, their interaction and proposes an analysis framework. The levels of decision making for adoption of health technologies have increased with decentralization and decision makers are facing complex multiple choices with restricted budgets. Decision makers themselves are interrelated in complex and creative systems determined and confounded by multiple factors. This framework is drawn from the medical technology deterministic approach to the new theory of technology in practice, emphasizing each level actor's own interests, from patients, physicians, health care industry to health care managers. Health care product or program technology adoption decision-making processes are influenced by the application and intensity of those factors, consequently shaping and modifying the local, regional and national health care installed capacity. Empirical observation of adoption decision making within health care services of Brazil are an important research challenge requiring further efforts. Structured knowledge and measurement of such variables may help to develop improved proactive health care planning.
