Abstract
Background: 70% of global cancer mortality occurs in low and middle income countries (LMIC)1, yet less than 20% of Cochrane reviews are authored by people from LMIC. How well served by Cochrane cancer systematic reviews are populations of countries such as Iran?
Objectives: To identify the relevance of Cochrane reviews in cancer to cancer priorities in Iran.
Methods: We identified the 11 highest ranking cancers in Iran in terms of (a) incidence and (b) mortality in both male and female based on the data of the Globocan database 20022, WHO estimates for cancer3, and evidence published by cancer experts from Iran4 and categorized the Cochrane reviews published in Issue 1, 2007 to the identified 11 cancer disease by prevention, screening, diagnosis, treatment, supportive care and psychosocial and palliative care.
Results: The 11 cancer priorities in Iran are gastrointestinal, esophageal, stomach, colorectal, bladder, lung, leukemia, cervix uteri, endometrial, breast cancers. After categorizing the Cochrane reviews for each cancer priority, we found out that the following topics were not covered by completed Cochrane reviews:
1.gastrointestinal cancer: screening, diagnosis, psychosocial and palliative care.
2. esophageal cancer: prevention, screening, diagnosis, supportive care, psychosocial and palliative care.
3. colorectal cancer: diagnosis
4. bladder cancer: prevention, screening, diagnosis, supportive care, psychosocial and palliative care.
5. lung cancer: screening, diagnosis.
6. leukaemia: prevention, screening, diagnosis, psychosocial and palliative care.
7. breast: diagnosis
8. metastatic breast: prevention, screening, diagnosis, supportive care.
9. cervix uteri: diagnosis, psychosocial and palliative care.
10.endometrial: prevention, screening, diagnosis, supportive care, psychosocial and palliative care.
Conclusions: There is a need for more Cochrane reviews addressing topics that are the priorities of low and middle income countries like Iran. Facilitating the preparation of these reviews will require a systematic approach and collaborative effort.
