A triangulated approach for evidence-based practice in vulnerable populations with high blood pressure

Abstract
Background: High blood pressure (HBP) is a lifelong disease and a key risk factor for cardiovascular disease, which is the leading cause of death worldwide. HBP is one of the most preventable causes of these premature deaths.
Objectives: This longitudinal exploratory-descriptive design used both quantitative and qualitative methods to answer the following research question: what nursing practice components related to health promotion in vulnerable adult clients with HBP in community settings are recommended by research, practice and experts’ opinions?
Methods: This research triangulated design included three phases. The first phase was to collect the most relevant and the best available sources of the evidence using an integrated systematic approach in order to describe recommended concepts of nursing practice from a systematic literature review related to health promotion with adults with HBP. The second phase included a secondary data analysis from an aggregated client database as coded in the Omaha System. In the final phase a qualitative focus group method was used to clarify and maximize further the validity of the findings of the quantitative analysis.
Results: Qualitative method using focus group interviews was applied to explore nurse practioners’ opinions related to the quantitative findings. Five patterns emerged from the qualitative data analysis:
1. establishing rapport with the vulnerable population;
2. health education as central to health promotion;
3. holistic approaches for adherence to medication regimen in the vulnerable population;
4. more health problems with younger clients;
5. factors affecting nursing services.
Conclusions: In addition to qualitative and quantitative methods, through integrated literature review, synthesis tables for nursing interventions were developed for evidence-based nursing practice. The qualitative research approach is a means of exploring the depth, richness, and the inherent qualities of the complexity in the evidence related to health promotion in vulnerable populations with HBP. This qualitative approach supported and provided some validation of the quantitative data analyses.
