Abstract
"Introduction: The lack of a consistent and effective assessment tool for older adults with dementia in home care in Taiwan, and the inability of home care nurses to provide pharmacological and non-pharmacological pain management, may affect the quality of life of patients with dementia.
Objectives: The aim of this project was to promote evidence-based practice in the pain assessment and management of patients with dementia in community.
Methods: The project was implemented using the JBI Practical Application of Clinical Evidence System. Eight audit criteria representing best practice recommendations for pain assessment and management of patients with dementia in community were used. Through a Getting Research into Practice analysis, we identified three barriers to and facilitators of changing practice.
Results: This project was performed at five home care institutions. 14 home care nurses, and 109 patients with dementia participated into this project. Postimplementation audit compliance rates improved. Specifically, the compliance rate increase from 14.29%, 1.83%, 1.83% to 100% fir criteria 1, 2, and 3 respectively; from 0% to 100% for criteria 4 – 8 respectively. In addition, the average score of the home care nurses' perception of pain care for patients with dementia increased from 44.29 points before the interventions to 82.86 points, and all 14 home care nurses achieved a score of 100 points in the OSCE scenario-based exercises, which was a good learning outcome. "
