Proposals for guideline development groups "Dissemination of clinical practice guidelines and Assessment of Quality Improvement of Medical Care" in Japan

Abstract
Background:
The purpose of the clinical practice guidelines (CPG) is not only to publish evidence-based, fair, and credible CPG, but also to ensure that CPG are used practically in the field to solve important clinical issues and improve patient outcomes. Furthermore, it is also expected to evaluate the effectiveness of improved outcomes and lead to the next revision of the CPG.
Methods:
In order to make proposals for the guideline development groups (GDG) regarding the dissemination and implementation of clinical practice guidelines, we convened five pioneering practice guideline development groups that are working on "dissemination and improvement of the quality of medical care" as part of our EBM promotion project in Japan (Minds project), and organized the "Effectiveness Evaluation Study Group". Based on the discussions held at the Study Group, we have summarized our recommendations for GDG.

0)It is important for GDG to keep in mind the processes indicated in 1 )- 4) below as they work through CPG development stage.
1-(1) Dissemination of medical practice guidelines.
1-(2) Evaluation of dissemination.
2-(1) Evaluation of the implementation of the recommended practice.
2-(2) (If the recommendation is implemented) Evaluation of improvement in patient outcomes.
2-(3) (If the recommendation is not implemented) Evaluation of the reasons why the recommendation was not implemented and the validity of the recommendation.
3) (If the recommendation is implemented) Integration of new evidence.
4) Revision of CPG.
Conclusion
What is especially important now is to educate GDG that the guideline development process is a continuous series of development, dissemination, utilization, and evaluation efforts.
