Shaping the conceptual framework of the WHO Abortion care guideline

Abstract
Background- Abortion Care Guideline (2022) is an updated consolidated guideline, which replaces all the previous WHO guidelines on abortion care. This updated guideline reflects recent evidence on clinical management, service delivery, and law and policy related to abortion care. In addition, human rights was an important component to integrate.
Objectives: To synthesize the recommendations of the Abortion care guideline that efficiently integrates the multiple domains of quality comprehensive abortion care
Methods: Adhering to the WHO guideline development process, evidence review and draft recommendations were undertaken in three domains: law and policy, clinical service and service delivery. Integration of key human right principles was also performed. These recommendations were then finalized by the Guideline Development Group. A conceptual framework was developed that interlinked these domains so that the recommendations were presented in a cohesive manner.
Results: A conceptual framework and a digital platform were created to reflect the interlinkages of the recommendations to comprehensive abortion care. Firstly, this framework recognizes and acknowledges the needs of all individuals with respect to abortion by putting the woman and her values and preferences at the center. The information and recommendations are presented that reflect the continuum of abortion care: services applicable across the continuum of care, pre-abortion, abortion and post-abortion. As the individual moves through the abortion care pathway, the guideline provides the following aspects: “what” (specific clinical interventions), “who” (which type of health worker can provide the intervention, or self-management), “where” (locations where services can be provided), and “how” (service-delivery models, implementation considerations). Underpinning this framework, is the enabling environment, which is the foundation of quality comprehensive abortion care. This conceptual framework was then translated to the digital platform to provide a visual connection between the recommendations.
Conclusion: From the evidence synthesis to the final end product of this consolidated guideline, this conceptual framework and the interactive digital guideline serve as an example of how the multiple facets to a health issue can be presented.
