Use i-PARIHS framework to develop evidence implementation strategies for central venous catheter maintenance: a multisite quality improvement project

Abstract
Background: A better level of evidence-based nursing practice has been found to be effective in reducing the complications of central venous catheters (CVCs). The Integrated Promoting Action on Research Implementation in Health Services(i-PARIHS) was considered an ideal theoretical instrument to identify facilitator and barrier factors when making efficient evidence implementation strategies.
Methods: We used the i-PARIHS as a theoretical framework to identify the barriers and facilitator based on the results of the baseline audit in pediatric intensive care units of 6 Chinese tertiary children's hospitals. We compiled a set of specific implementation strategies and conducted a pre-post study to evaluate the preliminary effectiveness of the implementation strategies.
Results: Facilitator and barriers factor at the innovation, recipient, and context levels were identified respectively, and based on the results, a targeted CVC maintenance evidence implementation strategy was developed to apply the evidence to nurses' clinical work. Of the 22 audit criteria, most (17 of 22) showed significant improvements compared with the baseline data.
Conclusions: The i-PARIHS is an effective tool for developing targeted scientific implementation strategies. Evidence was applied to the clinical setting through a well-thought-out implementation strategy. The quality of the nurses’ clinical practice improved during CVC maintenance. However, there is no certainty that such positive results can be maintained, and long-term data are needed to verify this.
