Establishing an evidence-based nursing guideline of hepatic encephalopathy

Abstract
Background: Hepatic encephalopathy is one of the common complications in patients with liver cirrhosis, with an incidence rate of approximately 30 to 45%. That's not only reduces the patient's quality of life, but also imposes great physical, psychological and financial burdens on family members.
Objectives: The aim of this study is by the evidenced model construction the health care evidence-based nursing guideline of hepatic encephalopathy.
Methods: This study has divided into free phases: (1) the first phase of the use of systematic literature review method and meta-analysis to draft of the health care evidence-based nursing guideline for hepatic encephalopathy, (2) the second phase of an expert focus group research method for collect staffs’ and patients’ point of health care evidence-based nursing guideline for hepatic encephalopathy to conduct validity, (3), the third phase survey the feasibility of health care evidence-based nursing guideline for hepatic encephalopathy.
Results: A total of 12 systematic reviews, 1 randomized controlled trial, and 8 clinical care guidelines were included. There were 100 questionnaires for the feasibility of clinical nursing staff. The response rate was 94%. Finally, guidelines total 66 items are divided into nursing assessment, medication care guidance and monitoring, sodium restriction, weight management and water restriction, nutrition assessment and guidance, prevention of sarcopenia, and management of risk factors.
The evidence-based nursing guideline of hepatic encephalopathy including systematic literature review articles of evidence strength Level I and Level II, and clinical guidelines covering Grade A and Grade B, and organizes expert focus groups to conduct effective research. Through a complete and rigorous verification process, an empirical nursing guideline for hepatic encephalopathy with an evidence and in line with the national conditions and culture of the country has been established.
Implications for practice: Expect by this study results to provide healthcare services empirical and scientific advice, to enhance the quality of life of liver cirrhosis patients.
